A plain guide to who to see first: counsellor, psychologist, or psychiatrist. What each door is for, when medication is worth considering, and how to book without a referral.

Short answer. Start where the problem lives. When your body is running away with you and medication might matter, try a psychiatrist; for patterns and choices, try a counsellor or a psychologist. You can book a psychiatrist directly in India.
In short:
I am a counsellor in Bengaluru. In my room we talk through patterns, choices, and habits; we notice what helps, what hurts, and we try small, testable changes in the way your week actually runs. Medication changes the body. A psychiatrist is a medical doctor who can prescribe and review medicines, order tests where needed, and watch for side effects alongside whatever else is happening with your health today.
Psychologists do talking work too, and some offer assessments. They differ widely: a counselling psychologist may focus on life patterns and skills, while a clinical psychologist may add formal testing and structured treatment plans that include homework, exposure, or cognitive experiments. Neither is a medical doctor, and neither prescribes medication.

Start with your gut. When sleep has collapsed, your heart will not slow, or harm thoughts keep circling, begin with a psychiatrist and include medication in the first talk so your body has support. Call Tele-MANAS on 14416 immediately if danger feels close. For decisions, relationships, or work, begin with a counsellor or a psychologist and we will map the stuck places and start small experiments.
Unsure whether your body or your mind is louder? This piece may help you sort signals: Am I depressed or anxious?. If work has chewed through you for months and you look like a ghost to yourself, read Burnout and depression look the same from the inside; that distinction can steer the first appointment.
No referral is needed. You can book directly with a psychiatrist in India; people start when sleep, appetite, panic, or energy are broken, then add counselling to rebuild habits and relationships once things steady. For options beyond your neighbourhood, Amaha lists psychiatrists and runs a psychiatric hospital in Bengaluru; Mindtalk / Cadabam's offer 24/7 psychiatric and inpatient infrastructure in Bengaluru; DocVita is a marketplace that lists psychiatrists across 17 languages.
People often do both. Counselling can run alongside medication so you get steadier sleep while you also practise different decisions at work or in your relationship, and the two supports do not cancel each other. I am not a doctor, and I cannot prescribe; if medication is worth considering I refer you to a psychiatrist, and we continue talking if you wish.
Some needs require other rooms. Immediate risk belongs with emergency services, a psychiatrist, or Tele-MANAS on 14416; that includes plans to harm yourself, hallucinations, withdrawal, or a reaction to medicine that turns your body wild. If you need a formal diagnosis, or a medication review, book with a psychiatrist or a clinical psychologist; my room does conversation and behaviour change work. I work with adults roughly 25 to 45.
Couples work belongs in a room designed for both of you. Try Untangle or Heart It Out, which focus on relationships and tend to hold the heat better than a general conversation slot. If you want a space that is lighter than counselling, I also run a small cafe gathering in HSR; five or so people meet, it is not counselling and the day moves week to week.
First chat is free. A conversation is thirty minutes and booked through a short form; we can meet on video or in person in Bengaluru, in English or Hindi, at a time you choose. Rafiq is the practice at heyrafiq.com; the name comes from my podcast running since 2020, and it signals a friend who walks with you. I have been practising since 2015 and have worked with more than 140 people.
Counselling with me is plain and organised. I do not diagnose and I do not fix you; we look at behaviours, stories, and trade-offs, and we test different moves in the week ahead. If you want a sense of the messy first hour, read Why am I anxious after my first session?.
Call 14416 if overloaded. Tele-MANAS is India's 24/7 mental health helpline; you can ask for crisis support or guidance on where to go next, and they stay on the line while you find help. YourDOST is often free through an employer or a university and works in 20+ Indian languages. 1to1help is an employer-provided counselling service that is free to the employee.
Comparing many named people helps. TalktoAngel, Practo, and TheMindClan are directories where you can browse and compare; DocVita is a marketplace with psychiatrists and counsellors across 17 languages. MindPeers lists an RCI-credentialed tier at roughly Rs 2,100-2,400 a session; Manastha around Rs 1,112 a session in a bundle; BetterLYF roughly Rs 715-999; these were publicly listed and they move.
Want something on your phone first? Wysa is an Indian AI-first mental health app that chats with you and can escalate to a human when needed. If you want to compare approaches, try Which therapy is best for mental health?. Begin wherever your attention returns after skimming tonight.
Do I need a therapist to see a psychiatrist?
No. In India you can book a psychiatrist directly. People often start there when sleep, appetite, panic, or energy are in pieces. Counselling can run alongside medication if that helps you hold changes in real weeks.
How do I know if medication is worth considering?
Look at your body. When sleep, appetite, panic spikes, or energy swings are making daily life wobble, it is reasonable to see a psychiatrist. You are not signing a lifetime contract. If medication is not needed, they will say so plainly.
What does a psychologist do that a counsellor does not?
A psychologist may add formal assessments and structured treatment plans; a clinical psychologist can provide diagnosis. Counsellors and psychologists both do talking work and behaviour change. Neither is a medical doctor, so neither prescribes medication.
How many sessions will I need?
There is no universal number. We decide together as we go. Some people pause after a few meetings and return later. Others keep a standing slot for a while. I do not promise timelines or outcomes; we keep it honest and review openly.
What if money is tight right now?
Check YourDOST through your employer or university. 1to1help is free to employees with that benefit. Online options list roughly priced sessions: MindPeers RCI-credentialed tier around Rs 2,100-2,400, Manastha around Rs 1,112 in a bundle, BetterLYF roughly Rs 715-999. For crisis, call 14416.
